Brockley Court is a home for the Elderly Mentally Infirm 65 years plus. We aim to provide all the necessary care and attention for those who wish to spend their retirement in a secure and caring atmosphere. Brockley Court Home cannot care and accommodate for persons who require nursing care that cannot be managed by Community District Nursing Services.
We aim to give our residents the sort of life they would expect if living on their own or with their families, while at the same time providing them with care and support. We treat all our resident’s as individuals and show the dignity and respect that we expect of ourselves. Individual care plans are prepared with each resident and resident’s representative wherever possible. This enables us to develop an all round comprehensive view of the residents physical, spiritual and emotional needs.
The care management process is based on the following basic elements: v A key-worker will be designated to you. As a team they will be directly responsible for your individual personal and social care needs. They will also be responsible for ensuring that Care Plans are reviewed monthly and re-assessed bi-annually. v The monthly updates and bi-annual re-assessments will be conducted through consultation with you and/or your relative or representative. v Regular risk assessments on self-caring skills and autonomy will ensure maintenance of independence as far as is reasonably practicable. v The resulting Care Plan will be made available to you if desired.
